About the Student Referral Process

Columbia Regional Program is a support service to schools districts and must have a formal referral in order to initiate any evaluation process toward determining eligibility for services that Columbia Regional Program provides.


Who can make the referral?

Only school district personnel, with approval from the Special Education Director, may make a referral for Columbia Regional Program services. Parents cannot request CRP services on behalf of the student.


What is needed to make a referral?

* Read Sections A and B of the Process for Completing the CRP Referral
* Complete the online Columbia Regional Program Referral Form. Requires Adobe Acrobat Reader 7 or greater.
* Print the form.
* Secure the Special Education Director’s signature.
* FAX or mail the completed form to us.

Important : Incomplete forms or those lacking a Director or designated signature will be returned to the requester.


How long does the referral and evaluation process take?

The evaluation shall be completed within a reasonable period of time as defined by Law. For more information, refer to the Oregon Administrative Rules

Once CRP receives the referral form, we will work with you and your school to complete the student’s file so that any appropriate assistance can begin as soon as possible.
