Students must be referred by school district personnel. For details, please see our Student Referral: General Information page.
When referring students suspected of having a Hearing Impairment, please include a copy of an audiogram to assist in the evaluation and eligibility determination process. If you are referring a child in the birth-to-5 range, a copy of two failed screenings is required.
