About the Student Referral Process
A Columbia Regional Program Referral must be submitted to request an evaluation and/or initiate services for a student.
Who can make the referral?
Only school district personnel, with approval from the Special Education Director, may make a referral for Columbia Regional Program services. Parents cannot request CRP services on behalf of the student.
What are the steps to refer a student to CRP?
1. Complete and print the Columbia Regional Program Referral form.
2. Obtain the signature of the special education director (or designee) in your district.
3. Fax or mail the completed referral to CRP.

Please ensure all required fields are completed, and that any necessary. Referrals that are incomplete, do not have a director signature, or are not accompanied by required paperwork will be returned to the requestor.

What happens after I have submitted a referral?
Once CRP receives a completed referral and supporting documents, we will contact you to coordinate next steps. If you have questions regarding that status of a referral, please call us at 503-916-5570 and select the corresponding department.
